This JournalCast reviews the Editor’s Choice paper from the May 2026 issue of RESPIRATORY CARE, identifying patients at risk for unplanned extubations in the pediatric ICUs.
Participants will learn how to design and implement a structured risk assessment tool to identify pediatric ICU patients at high risk for unplanned extubation. They will understand the key risk factors included in the scoring system, how to ensure reliable use across clinicians, and how targeted mitigation strategies—such as rapid communication and interdisciplinary huddles—can reduce adverse events. Additionally, participants will gain insight into the tool’s strengths (high inter-rater reliability and association with fewer extubations) as well as its limitations in predicting which patients will experience an event.
