Effective Discharge Planning- Moving from Acute Care to the Community


Our goal in caring for patients, should be to return them to the community with the tools and equipment to prevent readmission to the acute care setting. In many cases this is easier said than done. We will discuss “best practices” for effective discharge planning, to decrease costly readmission. A team of individuals are necessary to ensure an effective discharge. Each team member must be aware of their role, and the roles of others. Best practices are often adopted to the local situation so we will discuss the barriers to effective discharges and some proven practices to prevent recidivism.

Successful completion of this course requires a score of 70% on the post-test.
